Dr. John Tate, MD
Dr. Tate earned his degree from Louisiana State University (LSU) in Baton Rouge, and was accepted into LSU’s School of Medicine in Shreveport where he was awarded his doctor of medicine. He remained at LSU’s Health Sciences Center for both his internship and residency in Otolaryngology/Head and Neck Surgery. He completed a year-long fellowship in Heiden, Switzerland, and Shreveport, La., under the direction of the American Academy of Facial Plastic and Reconstructive Surgery. He is board certified by the American Board of Otolaryngology/Head and Neck Surgery.
Leslie Allen, MSN, APRN, FNP-C
Leslie Allen is a native to West Tennessee, but has called Gallatin home for the past 17 years. She graduated in 1993 from Baptist College of Health Sciences in Memphis, TN and obtained her RN. After graduation, she started her career as a Registered Nurse on an ICU-step down unit at Baptist Hospital-Memphis and enjoyed working in the in-patient setting for one and half years. Her passion for the ENT field began when she became employed at an ENT practice in Memphis in 1994 and is still her passion today. She has had the unique experience to work with ENT physicians in Memphis, Minneapolis, and Nashville area.
Leslie completed her Bachelor of Science in Nursing from Cumberland University. In May 2014, she completed her Master of Science in Nursing from Western Kentucky University. She is board certified through the American Association of Nurse Practitioners Certification Program as a Family Nurse Practitioner and is a member of the Sigma Theta Tau International Honor Society of Nursing, Chi Epsilon Lambda Honor Society, and Pinnacle Honor Society. Leslie’s philosophy is: building partnerships, patient education, and open communications are the cornerstones of a good provider/patient relationship.
In her free time, she enjoys spending time with her family. Her hobbies are reading, boating, and traveling.
Mitzie Slayden, Au.D., CCC-A
Dr. Mitzie Slayden has been an audiologist for thirty years. She earned her Bachelor of Science in Communication Disorders from Murray State University in 1980. In 1986, she completed her Masters in Audiology at the University of Memphis. She later returned to Salus University to obtain her Doctorate in Audiology and completed in 2009. During her career, she has had the opportunity to work in private practice settings and with hearing aid manufacturers. As an outside representative with a manufacturer, she worked with audiologists in their offices training them on the latest technology as well as helping ensure successful hearing aid fittings with patients with challenging hearing losses. She applied this knowledge to helping patients when she began her own private practice. Over the years, she has helped thousands of hearing impaired people to hear better. Mitzie believes she is providing her patients with a better quality of life by improving their ability to communicate with their friends and family.
Mitzie grew up in Gallatin and Clarksville and has lived in Mount Juliet since 2006. She shares her home with her “fur baby”, Honey, a Malti-poo that she has spoiled. She is an avid reader and enjoys seeing movies, Broadway shows and musicals, and attending live music venues. Traveling to new places whether near or far, flying to a new country for vacation, or a quick road trip on the weekend is her favorite way to spend her free time.
